Key takeaways
  • Conch piercings take 6-12 months to fully heal — thick cartilage with limited blood supply
  • Inner and outer conch heal at similar rates
  • The flat cartilage location makes sleeping pressure a major healing challenge
  • Switch to a hoop no earlier than month 6, ideally month 9 with piercer approval
  • Over-ear headphones can cause issues — switch to earbuds during healing

Healing timeline

Day 1-3
Acute inflammation
The flat cartilage area swells noticeably. Throbbing warmth. Clear discharge. The ear may feel stiff. All normal.
Week 1-2
Peak swelling subsides
Crusty discharge forms around the stud. Do not pick at it. Saline softens and removes it. Sleeping on this ear is painful — avoid.
Month 1-3
Early healing
Discharge reduces. The conch feels more settled. Occasional flare-ups from sleeping pressure or headphone contact are common.
Month 3-6
Intermediate
Looks healed externally. Internal channel is still forming. Do not attempt the stud-to-hoop switch yet. A downsize of the stud post may be appropriate.
Month 6-9
Approaching maturity
With piercer approval, you may be ready for a hoop switch. Some piercers prefer waiting until month 9 for maximum channel maturity.
Month 9-12
Fully healed
The channel is established and robust. Hoop switches are safe. The piercing tolerates regular jewellery changes and normal daily activity.
Sleeping is the biggest challenge
The conch sits on the flat surface of the ear, making it directly exposed to pillow pressure. This is the number one cause of conch healing complications. Use a travel pillow with a hole, or train yourself to sleep on the opposite side for 6+ months. This is non-negotiable for smooth conch healing.

What is normal

Clear or pale discharge: lymph fluid. Healthy and expected.

Crusty buildup: dried lymph around the stud. Normal for months.

Intermittent swelling: especially after sleeping pressure or headphone use.

Mild itching: tissue regeneration in progress.

Warning signs

Green or foul-smelling discharge: possible infection. See a doctor.

Increasing pain after week 2: should be decreasing, not escalating.

Spreading redness: beyond the immediate piercing area suggests infection.

Persistent bump: likely irritation from pressure, headphones, or poor jewellery. Remove the cause.

Aftercare protocol

Saline twice daily. The conch is relatively easy to clean — spray directly onto both sides of the stud.

No sleeping on it. Travel pillow or opposite-side sleeping for 6+ months.

Headphone awareness. Over-ear headphones press on conch piercings. Use earbuds or bone conduction headphones during healing.

Watch for snagging. Scarves, turtlenecks, and hair can catch on the stud. Be mindful when dressing.

No swimming for 4-8 weeks minimum.

When to switch to a hoop

Minimum: month 6. Only if the channel shows zero tenderness and no discharge.

Ideal: month 9. The channel is mature and stable at this point for most people.

Always: have your piercer perform the first hoop insertion. They can confirm readiness and ensure correct sizing.

Frequently asked questions

Why does my conch keep getting bumps?
Sleeping pressure and over-ear headphones are the top causes. The flat location is vulnerable to sustained pressure. Remove the cause, strict saline care, and 2-4 weeks of patience resolves most bumps.
Can I use over-ear headphones with a conch?
During healing: avoid them or use very sparingly. They press directly on the conch area. Once healed: over-ear headphones are generally fine, though some people find they push against the hoop. Earbuds are the safe alternative during healing.
Does inner or outer conch heal faster?
They heal at similar rates (6-12 months). The cartilage thickness and blood supply are comparable in both areas.
