Key takeaways
  • Standard helix: 3-6 months healing. Forward helix: 6-12 months
  • The helix heals faster than most cartilage piercings because the cartilage is relatively thin
  • It will look healed weeks before the internal channel is fully formed
  • The most common mistake is changing jewellery too early (before 3 months for standard helix)
  • Saline twice daily, no touching, and no sleeping on it are the foundations

Healing timeline

Day 1-3
Acute inflammation
Swelling, redness, warmth, and throbbing. Clear or pale yellow discharge (lymph fluid) is normal. Do not touch.
Week 1-2
Peak swelling subsides
Crusty discharge forms around the jewellery — dried lymph, normal healing sign. Saline will soften and remove it.
Month 1-2
Settling in
Discharge reduces. The piercing becomes less actively noticeable. Occasional twinges when bumped are normal.
Month 2-4
Looking good
Appears healed externally. The internal channel is still maturing. Resist the urge to change jewellery. A downsize from your piercer may be appropriate.
Month 4-6
Standard helix: healed
Most standard helix piercings are functionally healed. With piercer approval, switch to a hoop.
Month 6-9
Full maturity
The channel is fully established. Forward helix piercings may still be healing at this stage.

Do not apply standard helix timelines to forward helix
This is the most common mistake. A standard helix may be ready for a jewellery change at month 3-4. A forward helix at month 3-4 is still in early healing. Treat them as entirely different piercings with different timelines.

What is normal during healing

Clear or pale yellow discharge: lymph fluid. Healthy and normal. Dries into crusts around jewellery.

Occasional swelling flare-ups: can happen for months, especially after being bumped or slept on.

Itching: a positive sign of tissue regeneration. Do not scratch.

Slight redness around holes: normal for weeks to months. Cartilage heals slowly.

Warning signs

Green or dark yellow discharge: possible infection. See a doctor.

Increasing pain after week 2: pain should decrease, not escalate.

Red streaks spreading from piercing: possible spreading infection. Seek medical attention.

Persistent bump: likely irritation bump. Identify and remove the cause (sleeping on it, touching, poor jewellery).

Aftercare protocol

Clean twice daily with sterile saline solution (0.9% NaCl). Spray, wait 30 seconds, pat dry.

Never touch with unwashed hands. Do not twist or rotate jewellery.

Do not sleep on it. Use a travel pillow for 3-6 months.

Keep hair products away. Hairspray, conditioner, and styling products irritate healing piercings.

No swimming for at least 4 weeks.

Frequently asked questions

When can I change my helix to a hoop?
Standard helix: 3-6 months minimum. Forward helix: 6-12 months minimum. Always get your piercer to assess before your first change.
Why does my helix keep getting irritation bumps?
Common causes: sleeping on it, touching it, poor jewellery quality, hair products, and premature jewellery changes. Remove the cause and return to strict saline care. Most bumps resolve in 2-4 weeks.
Can I speed up helix healing?
You cannot accelerate biology, but you can avoid slowing it down. Good nutrition, adequate sleep, no smoking, consistent saline care, and not touching the piercing give your body its best chance.
